What Support Coordination Looks Like
Step 1: Understanding and Implementing Your Plan
We begin by reviewing your NDIS plan in detail, helping you understand your support budgets and how they can be used. We then work with you to identify your needs, set up service agreements, engage new providers, and make sure your supports are working together to help you meet your goals.
Step 2: Monitoring and Reporting
Throughout the duration of your plan, we continue to monitor how things are going. If any issues arise or changes are needed, we work with you to adjust services, resolve concerns, and keep your support network aligned. We also prepare regular progress reports to share with the NDIA, ensuring your plan is well-documented.
Step 3: Reviewing Your Plan
When your plan is due for review, or if your circumstances change and you need a reassessment, we help you prepare the necessary documents and reports. We ensure that your voice is at the centre of the review process and that your plan reflects your evolving needs and aspirations.

Levels of NDIS Support Coordination
The NDIS provides three levels of support coordination depending on your needs and circumstances:
Support Connection
Designed to help you connect with informal, community, and funded supports so you can make the most of your plan.
Support Coordination
Ongoing coordination to help you understand and implement your NDIS plan, manage your services, develop independence, and participate actively in your community.
Specialist Support Coordination
For participants with more complex or high-risk situations. A Specialist Support Coordinator works closely with your providers and support network to overcome barriers and stabilise support arrangements.
We offer both Support Coordination and Specialist Support Coordination for participants across a range of circumstances and needs.
